Emergency Care
In an emergency, please dial 911.
Telehealth Appointments
Patients are seen Monday through Friday by appointment from 8 am to 5 pm. Other times can be arranged if necessary. We ask that you call in advance to schedule a convenient appointment time. Every effort will be made to keep appointment commitments. Whenever possible, you will be notified if there is a change or delay. If you cannot keep your appointment, we ask that you notify us at least 24 hours in advance.
Prescription Refills
We ask that all prescription refills be requested during your office visit. Please assess any refill needs before coming to the office. Please allow 48 hours for your request to be processed.
Telephone Calls
Please call our office if you have any questions regarding your condition, medication, or treatment. Our office staff can also assist you with questions regarding appointments. We ask that you always spell your name when leaving messages and give your date of birth. Please remember our team is caring for patients throughout the day - we value your concerns - and will get back to you at our earliest convenience, which may be at the end of the working day.
Referrals
Please feel free to call our office if you have any questions. Sometimes, a referral is needed for insurance and continuity of care. We ask that you call the office at least 72 hours in advance.
Patient Services
We participate with many insurance companies. Please confirm with your insurance company that we are participating providers. We will collect your deductible, co-payment, and fees for non-covered services, along with any balances at the time of your visit. We MasterCard, Visa, American Express, and Discover Card.

Medical Records
Your medical records are completely private. To ensure patient confidentiality and conforming to HIPAA regulations, this office will release no information regarding your condition to anyone, including any family member, without your written consent.
